I understand that by choosing "I accept", the photo submitted will be included in a video that will aired on a social media platform (YouTube), and which will be available for viewing by the public, and give permission for the photo to appear in the video, along with my loved one's first and last name. By choosing 'I do not accept' the photo submitted will not be included in the tribute video and will not appear during the Tree of Life video tribute.

Avoid photos with text, stickers or any additional markings. Please avoid sending photo collages or files containing multiple photos.
Media/Photo Release For Tree of Life Open House*
I, (individual, representative or guardian) hereby authorize the shooting and releasing of photographs and/or video recordings taken during the Tree of Life Open House on May 23, 2025 by LifeLink employees or by any third parties involved. I understand that these images may be used for promotional purposes, including but not limited to social media, websites, and printed materials. This agreement is intended to release all LifeLink, its employees, and any third parties involved in the creation or publication of the photographs and/or video recordings from any liability related to the use or publication of said media for any non-commercial use. I waive any right to inspect or approve the final copy, photographs or video that may be used or the use to which it may be applied. This authorization is ongoing, and is without limitation or restriction to time. I HAVE FULLY READ THIS AGREEMENT BEFORE SIGNING AND I FULLY UNDERSTAND IT'S CONTENT.
Your Media/Photo Release selection DOES NOT affect whether you're allowed to attend the Tree of Life Open House event. We will have protocol in place to ensure we respect your decision.
